Absentee Slips

The university nurse gives absentee slips for medical excuses. Students are responsible for visiting the nurse when he/she is ill. If unable to visit the illness or accidents must be reported to Residence Hall Directors.

Accident Reports

University Nurse, Residence Hall Directors and the Athletic Department are supplied with forms to be filled out and submitted to Health Services. Accidents will be reported within 24hours to the University Nurse.
 

Daily Register

A daily register is kept on each student who receives services.

Health Forms

The University Nurse uses health forms to obtain and provide the best possible medical care to all students. 

Health Service Referral Forms

This form is to be completed by the University Nurse for outpatient and inpatient treatment, also to diagnose beyond the scope of care available through Health Services.  This form must be completed before services rendered in Health Services.

Accident and Sickness policy underwritten by K&K Insurance and the Mega Life Health Insurance for full-time enrolled students.
