This is the formal application for accommodations from the Office of Disability Services. This form is required in order to receive supports for your documented disability. You can download, print, and fill out at your convenience to bring to your intake appointment or you may wait until your intake appointment and fill one out at that time.
This form will be completed after you have provided the Office of Disability Services proper documentation of your disability. It outlines your rights and responsibilities as well as HT’s rights and responsibilities to you. It documents the accommodations you will receive and is signed by both you and the Coordinator of Disability Services.
This information explains the policies and procedures followed for students who have testing accommodations listed on their accommodations letters. This information will be reviewed with the student and Coordinator of Disability Services and then signed by the student. A paper copy of this agreement will be kept in your student file.
Testing Accommodation Tracking Form (Must be picked up at the ODS Office)
This form is to be filled out by the student for each exam taken during the semester. It details the student and class information, testing date, and time. It is the student’s responsibility to take this form to his/her professor a few days in advance of a scheduled exam so that the professor may fill in his/her information about how the test will be delivered to the Office of Disability Services and how it should be returned to the professor. After getting the professor’s information on the form, the student returns the form to the Office of Disability Services at least one day in advance of the exam. Students with testing accommodations will use many copies of this form during the semester.
If a student feels that his/her concerns are not being heard by staff in the Office of Disability Services or he/she disagrees with any decisions made there, he/she is encouraged to talk with the Coordinator of Disability Services. If the student does not agree with the conclusion of that discussion, he/she may speak with the Director of Counseling and Consultation about his/her concerns. If, after that conversation, the student still feels that his/her needs are not being heard, the student has the right to file a grievance according to HT policies and procedures. This grievance form may be downloaded and filled out or a hard copy may be requested in the Office of Student Affairs. A completed grievance form should be turned in to the Dean of the Office of Student Affairs. If the student has any questions about the proper procedure or needs help filling out the grievance form, staff in the Office of Student Affairs are available for assistance. The Office of Student Affairs is located in the Davage-Durden Student Union Building.
